What is the difference between effective and marginal tax rate?

Your marginal rate is the tax rate on your last dollar of income. Your effective rate is the average rate across all your income. For example, someone earning $100,000 in the US has a 22% marginal rate but an effective rate of around 15%.

Does this include state or provincial tax?

No, this calculator shows only federal/national income tax. In countries like the US, Canada, Germany, and India, additional state or provincial taxes apply on top of federal rates.
